COP4 Deputy's Declaration

The Court of Protection can make a deputyship order under which it appoints someone (called the deputy) to act and make decisions on behalf of someone who lacks the mental capacity to manage his own affairs (called the protected person)

The Court of Protection is concerned with ensuring that protected persons are not taken advantage of , and so where someone makes an application to the Court to be appointed as a Deputy, the Court will not grant a deputyship order unless it is satisfied that the applicant is a fit and proper person to act as a deputy.

This form should be used by the applicant for a deputyship order to provide the supporting information on which the Court will make its decision about his suitability.
