EP1PG Notice of Intention to Apply for the Registration of an Enduring Power of Attorney

A Power of Attorney document allows an individual (called the donor) to appoint people to act on his behalf (called the attorneys). Normally, a Power of Attorney becomes invalid if the donor loses mental capacity, for example as a result of illness or injury, and the attorneys are no longer able to act. An Enduring Power of Attorney (EPA) is a special document which remains valid even if the donor becomes mentally incapable.

The attorneys cannot act under an EPA until it has been registered with the Office of the Public Guardian. This is a safety measure designed to protect vulnerable donors from exploitation.  When the attorneys wish to register the EPA, they must use this form to notify the donor and his nearest relatives, who will then have an opportunity to object to the registration. If no objections are received, the attorneys may apply for the EPA to be registered using form EP2PG.
