EP2PG Application for Registration of an Enduring Power of Attorney

An Enduring Power of Attorney (EPA) is a document by which an individual (the donor) appoints others (the attorneys) to act on his behalf if he loses mental capacity or becomes unable to make decisions and manage his own affairs.



In order to protect donors from being taken advantage of, attorneys cannot act under an EPA until it has been registered with the Office of the Public Guardian. The Public Guardian is empowered to supervise and monitor attorneys in the discharge of their duties and to ensure that they are acting in the best interests of the donor.



Using this form the attorneys can make a formal application for the EPA to be registered, and this should be done in accordance with the Office of the Public Guardian's EPA guidance notes.
