LPA001 Notice of Intention to Apply for Registration of a Lasting Power of Attorney
A Lasting Power of Attorney is a document which allows one person (the donor) to appoint others (the attorneys) to act on his behalf when he is no longer able to make decisions for himself.
The attorneys cannot act until the LPA has been registered with the Office of the Public Guardian. When making a Lasting Power of Attorney, the donor will be asked to nominate two trusted people (usually relatives or close friends) who must be notified before an LPA can be registered. This is to protect the donor from being taken advantage of by his attorneys.
This form can be used to notify the relevant people that the attorneys wish to register the Lasting Power of Attorney with the Office of the Public Guardian. Those people will then have the opportunity to make objections, if necessary. It must be completed, and sent to all people who are named in the Lasting Power of Attorney as requiring notice of registration, and if no objection is received, then an application to register may be made using form LPA002.
