OPG100 Application for a Search of the Public Guardianship Register

The Office of the Public Guardian is a statutory body which was set up by the Mental Capacity Act 2005 and which is responsible for keeping records of all Enduring Powers of Attorney (EPAs), Lasting Powers of Attorney (LPAs) and Court of Protection Deputyship Orders which have been registered and are currently in force.



Members of the public can apply to search the register by completing this form and sending it to the office of the Public Guardian along with the appropriate fee. Full details of the application process and the fees charged can be found in the OPG guidance booklet.
