During the divorce process, when applying for a decree nisi, the petitioner must include with their application form a statement confirming that everything said in the petition is true. This form is for making such a statement when the reason for the divorce is adultery. There are different forms for all the five facts of divorce, so make sure you are using the right one.

After all the questions have been answered and the statement of truth has been signed, this form should be sent to the court with a copy of your spouse’s response to the divorce petition.
