D80E Statement in Support of Divorce/Dissolution/(Judicial) Separation - 5 Years' Separation

This form is for people who are in the midst of the divorce process and are applying for their decree nisi. When sent to the court, the decree nisi application form needs to be accompanied by a statement confirming that everything said in the divorce petition is true.

For each of the five facts of divorce there is a different statement form. This form is to be used when the reason on which the request for divorce is based is five years’ separation.

The form contains a number of questions which need to be answered, as well as a statement of truth which needs to be signed. Note that making a false statement verified by a statement of truth can result in contempt of court.

Once received, a judge will use the documents to decide whether or not a decree nisi should be granted.
